Arthrosis treatment
Arthrosis belongs to the so-called degenerative joint disease, which leads to reduced mobility of the joint and its subsequent damage. It is accompanied by great pain, stiffness and swelling of the joint. It most often affects large joints (shoulder, hip, knee), but it also often appears on small joints (hand joints).
Arthrosis
Although it mainly affects the older generation, its occurrence in connection with the lifestyle affects an increasingly younger population, e.g. athletes, patients after injuries or people with one-sided professional load.Early diagnosis of arthrosis and its degree makes it possible to indicate targeted treatment, whether conservative or operative, which aims to slow down the process of joint deformation and eliminate painful conditions.

Non-pharmacological treatment
Non-pharmacological treatment is indicated in the early stages of arthrosis and primarily includes lifestyle modification, weight reduction or the inclusion of regular stretching exercises.
A significant part of the treatment is individual rehabilitation care, which includes a whole range of methods: physical (exercises, massages - also possible at our physiotherapy workplace), magnetotherapy, laser or ultrasound therapy. An integral part is also the recommendation of proper footwear, prosthetic or support aids (crutches).
Pharmacological treatment
Pharmacological treatment aims to eliminate painful conditions and includes treatment with analgesics, opioids, non-steroidal anti-rheumatic drugs including local preparations (ointments, gels, patches), glucocorticoids, supportive preparations (SYSADOA) containing active substances such as chondroitin sulfate or glucosamine sulfate.
- Analgesic treatment, such as Paracetamol, reduces pain and is suitable for the early stages of the disease.
- In addition to pain relief, non-steroidal anti-rheumatic drugs also have an anti-inflammatory effect, e.g. ibuprofen, diclofenac, etc., but they also have a number of adverse effects on the digestive tract (stomach ulcers) or the circulatory system. For this reason too, the use of locally applied non-steroidal anti-rheumatic drugs in the form of ointments, gels or patches is recommended for arthrosis of the knee joint and especially for hand joints, where the resulting effect of the application is increased blood flow and subsequent pain relief.
- Intra-articular application of glucocorticoids has been used in the treatment of arthrosis for more than 50 years and is indicated especially for highly painful conditions accompanied by inflammation. The application is especially recommended in the knee joint and the thumb joint.
- SYSADOA are supportive preparations containing chondroitin sulfate, glucosamine sulfate, diacerein and ASU (avocado and soy extract), which by their effect support the formation of basic cartilage mass, increase cartilage elasticity and prevent its further loss. They are especially recommended for arthrosis of the knee or hip joint. Their effect begins within 2-4 weeks and the resulting effect persists for about 2 months after discontinuation.
- Hyaluronic acid, the so-called viscosupplementation, is recommended by the CRS (Czech Society of Rheumatology) for patients with osteoarthritis of the knee joint who have failed or are contraindicated in treatment with nonsteroidal antirheumatic drugs. Hyaluronic acid is the body's own substance, which is part of the intra-articular fluid. This fluid reduces the friction of the joint surfaces and thus reduces their wear. With intra-articular application, we supplement its insufficient production and support the production of our own joint fluid. Another significant effect is its anti-inflammatory and analgesic effect, which lasts up to 1 year after application. The health insurance company covers viscosupplementation for second and third degree arthrosis of the knee joint, however, a significant therapeutic effect can also be achieved when applied to other joints, e.g. small joints of the hand and foot. Operative treatment
Operative surgical treatment is indicated where there is a gradual progression of the disease and conservative treatment fails. It includes several methods. The most frequently indicated procedure is arthroscopic surgery, which is the first choice due to its wide application. Other treatment methods include corrective osteotomy and total replacement of the hip and knee joint.
Arthroscopy with treatment of the affected joint
Today, arthroscopy is one of the routine diagnostic and therapeutic endoscopic methods for a wide range of joint diseases, including arthrosis. Its aim is to relieve pain and improve joint function. With its mini-invasive approach, it contributes to minimal traumatization of the surrounding tissues and thus enables rapid recovery and return to full movement of the limb. The experienced team of our arthroscopic center performs approximately 1,200 arthroscopic procedures per year not only on large joints - knee and shoulder, but also on elbow, ankle and wrist joints. All procedures are performed under very gentle general anesthesia in a one-day care regime with a short post-operative bed rest. The care is covered by public health insurance. Arthroscopy of the knee joint
With arthrosis of the knee joint, not only the joint surfaces are affected and changed, but also the soft tissues of the joint - menisci and ligaments - are destroyed, which manifests itself as a visible deformity in the axis of the joint (bent or bent joint). Arthroscopic treatment in this case consists in treating the joint surface with so-called shaving, removing loose joint bodies, damaged parts of the menisci or irritating bone growths called osteophytes.
Arthroscopy of the shoulder joint
Osteoarthritis of the shoulder joint occurs mainly after injuries, inflammations, poor healing of fractures or rotator cuff syndrome. Arthroscopic treatment consists of treatment of the joint surface, removal of loose joint bodies, treatment of the rotator cuff, etc.
Arthroscopy of the ankle joint
Osteoarthritis of the ankle joint occurs in patients with repeated injuries that lead to injury to the stabilizing ligaments of the ankle and to excessive and disproportionate stress on the joint surfaces.In the case of the ankle joint, arthroscopic treatment consists in the treatment of cartilage defects, removal of loose articular bodies, bone growths, etc.
Osteotomy
Osteotomy is an operation that preserves the joint itself. By correcting the mechanical axis of the limb, the axis will be aligned, the load distributed and transferred to the healthy half of the joint. It is especially indicated for young and active people, for whom it significantly delays the time when it will be necessary to proceed with a total joint replacement. The advantage is that the patient returns to an active life, including sports, relatively quickly after healing. We perform the osteotomy at a contracted hospital workplace.
Total joint replacement (TJR)
Total joint replacements (hip and knee endoprosthesis) are common surgical procedures today. During TEP surgery, the head and socket of the affected joint are replaced with an implant. A number of types of replacements are used (uncemented, cemented, hybrid, Hip resurfacing), taking into account the patient's age, lifestyle, bone shape and strength.
