In order to be accepted for the procedure, a questionnaire with important data must be filled out, especially with regard to anesthesia.

The anesthesiology questionnaire is filled out by the patient, possibly in cooperation with the general practitioner. Using the questionnaire, we try to prevent even relatively minor risks, such as nausea, vomiting, tooth damage, voice disorders and phlebitis. Please, in the “Other noteworthy” paragraph, point out the circumstances that you think are important. In addition, it is necessary to arrange a pre-operative examination with a general practitioner or internal medicine doctor to the extent prescribed.
